Outcomes & Measures: (self-reported)

Outcome 1

Measure
Client able to leave home, make life easier and don't have to move from home

Outcome 2
Food delivered

Measure
Transportation barrier removed

Outcome 3
Fire wood delivered

Measure
Client able to heat the home

Change Process: (self-reported)

When a ramp is built, the client can leave their home again. When a minor home repair is done, it keeps the client independent. If food is delivered, the barrier of transporation is removed. We deliver wood for clients that use it to heat their homes to keep their utilities lower.
